L609
L609 – L609
OHIP Dermatology Code · Schedule of Benefits
When to Use
- Use L609 for the simple excision of a single skin tag or small benign lesion when no complex closure or pathology submission is required.
- Select L609 when performing a minor superficial procedure that does not meet the criteria for the more extensive excision codes like Z303 or Z304.
Common Pitfalls
- Billing L609 in addition to a minor assessment code (A007) for the same patient on the same day is often rejected as the assessment is considered included in the procedure fee.
- Attempting to bill L609 for multiple lesions on the same site will trigger a rejection; use the appropriate multiple procedure logic or specific excision codes if the lesion size exceeds the scope of L609.
Billing Tips
- Ensure the clinical note clearly describes the lesion size and location to justify the use of L609 over a standard office visit fee.
