When to Use

  • Use L609 for the simple excision of a single skin tag or small benign lesion when no complex closure or pathology submission is required.
  • Select L609 when performing a minor superficial procedure that does not meet the criteria for the more extensive excision codes like Z303 or Z304.

Common Pitfalls

  • Billing L609 in addition to a minor assessment code (A007) for the same patient on the same day is often rejected as the assessment is considered included in the procedure fee.
  • Attempting to bill L609 for multiple lesions on the same site will trigger a rejection; use the appropriate multiple procedure logic or specific excision codes if the lesion size exceeds the scope of L609.
