When to Use
- Use L638 for the excision of a single benign skin lesion, such as a skin tag or small seborrheic keratosis, when the procedure is performed via simple excision.
- Select L638 when the lesion is removed by simple excision rather than cryotherapy or cautery, which would be billed under different procedural codes.
Common Pitfalls
- Billing L638 in addition to an office visit fee (A007) is generally disallowed unless the patient presents with a separate, unrelated clinical issue that requires a full assessment.
- Submitting L638 for the removal of multiple lesions often leads to rejections or audit flags; ensure you are using the appropriate multiple-lesion modifiers if applicable.
