When to Use
- Use L647 for the excision of a skin lesion that is specifically identified as a benign lesion, such as a seborrheic keratosis or dermatofibroma.
- Select L647 when the procedure involves a simple excision of a single lesion, distinct from the more complex or diagnostic requirements of L648 or L649.
Common Pitfalls
- Billing L647 in conjunction with an office visit (A007) on the same day is generally disallowed unless the visit is for a separate, unrelated clinical issue.
- Submitting L647 for a malignant lesion is an error; malignant lesions must be billed under the appropriate excision codes (e.g., L650-L653) which carry higher values.

- Ensure the pathology report supports the benign nature of the lesion, as this is the primary documentation required to defend the use of L647 during an audit.
