When to Use
- Use L648 for the excision of a malignant lesion on the trunk or extremities when the lesion diameter is between 0.5 cm and 1.9 cm.
- Select L648 instead of L647 when the lesion is located on the trunk or extremities rather than the head, neck, or genitalia.
Common Pitfalls
- Billing L648 in conjunction with a minor assessment (A007) is prohibited as the assessment is considered included in the surgical fee.
- Submitting L648 for a benign lesion excision is a common audit trigger; ensure the pathology report confirms malignancy to support the code selection.
Billing Tips
- If the excision requires a complex closure, you may be eligible to bill an additional fee code for the repair, provided it meets the criteria for a separate procedure.
