When to Use
- Use L667 for the excision of a single benign skin lesion, such as a seborrheic keratosis or skin tag, when performed via simple excision.
- Select L667 when the procedure is performed as a standalone minor surgery rather than as part of a more complex procedure like a biopsy (L665) or a larger excision requiring complex closure (L668).
Common Pitfalls
- Billing L667 in conjunction with an office visit (A007) is generally disallowed unless the excision is a separate, identifiable service performed for a distinct clinical issue.
- Submitting L667 for the removal of multiple lesions in the same anatomical region without using the appropriate multiple procedure rules often leads to automatic claim adjustments or rejections.
