When to Use
- Use L679 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, when the procedure is performed via simple excision.
- Select L679 when the lesion is removed by curettage or cautery rather than surgical excision, provided the lesion is benign.
Common Pitfalls
- Billing L679 for the removal of a malignant or premalignant lesion, which should instead be billed under the Z-series excision codes based on size and location.
- Attempting to bill L679 in addition to a consultation or assessment code (A007/A001) on the same day without the appropriate 'C' prefix or diagnostic code justification for a separate procedure.
- Submitting L679 for multiple lesions in the same anatomical area without applying the appropriate multiple procedure reduction rules.
Billing Tips
- If removing multiple benign lesions, bill the first lesion at 100% and subsequent lesions at 50% using the appropriate suffix to avoid automatic rejection.
