When to Use

- Select L680 when performing a simple shave or scissor excision of a superficial lesion where the procedure time and complexity do not meet the criteria for a minor surgical procedure code like Z001.
- Apply L680 for the removal of a single lesion when the service is provided as a standalone procedure rather than as part of a comprehensive assessment or consultation.
Common Pitfalls
- Billing L680 in addition to a consultation or assessment code (A007/A005) for the same patient on the same day is generally rejected unless the lesion removal is unrelated to the reason for the visit.
- Submitting L680 for multiple lesions will lead to payment for only one unit; use the appropriate multiple lesion codes if applicable, or document the distinct anatomical sites if billing multiple units.
- Confusing L680 with Z001; L680 is for simple, superficial removals, whereas Z001 is reserved for procedures requiring more extensive surgical technique or formal pathology.
