Common Pitfalls

  • Billing L680 in addition to a consultation or assessment code (A007/A005) for the same patient on the same day is generally rejected unless the lesion removal is unrelated to the reason for the visit.
  • Submitting L680 for multiple lesions will lead to payment for only one unit; use the appropriate multiple lesion codes if applicable, or document the distinct anatomical sites if billing multiple units.
  • Confusing L680 with Z001; L680 is for simple, superficial removals, whereas Z001 is reserved for procedures requiring more extensive surgical technique or formal pathology.
