  • Do not bill L686 in conjunction with a minor assessment code like A007 for the same lesion, as the assessment is considered included in the procedure fee.
  • Avoid billing L686 for lesions requiring deep excision or complex closure, as these should be billed under the Z-series surgical excision codes which carry higher values.
