When to Use
- Use L686 for the excision of a single benign skin lesion, such as a skin tag or small seborrheic keratosis, that does not require formal pathology.
- Select L686 when performing a simple shave or scissor excision that is less invasive than the formal excision procedures covered under codes like Z001 or Z002.
Common Pitfalls
- Do not bill L686 in conjunction with a minor assessment code like A007 for the same lesion, as the assessment is considered included in the procedure fee.
- Avoid billing L686 for lesions requiring deep excision or complex closure, as these should be billed under the Z-series surgical excision codes which carry higher values.
