When to Use
- Use L702 for the excision of a malignant lesion on the face, scalp, neck, genitalia, or hands where the lesion diameter is between 1.0 and 2.0 cm.
- Select L702 instead of the minor surgery code G372 when the procedure involves a formal excision of a biopsy-proven or clinically suspicious malignancy requiring specific margins.
Common Pitfalls
- Billing L702 for benign lesions is a frequent audit trigger; use the appropriate Z-code series for benign excisions instead.
- Submitting L702 with an excision code for a different lesion on the same anatomical site during the same encounter will result in a rejection for duplicate service.
- Failing to document the exact measured diameter of the lesion can lead to a clawback if the Ministry determines the lesion size did not meet the 1.0 cm threshold.

- Ensure the pathology report is referenced in your billing notes to justify the malignant diagnosis required for this specific fee code.
