When to Use
- Use L708 for the excision of a single benign skin lesion, such as a skin tag or small seborrheic keratosis, that does not require a complex closure.
- Select L708 when performing a simple shave or snip excision of a superficial lesion where the pathology does not necessitate a formal surgical tray or extensive suturing.
Common Pitfalls
- Billing L708 in addition to a minor assessment code (A007) for the same patient on the same day is often rejected unless the lesion removal is unrelated to the reason for the visit.
- Submitting L708 for lesions requiring deep excision or complex suturing is an audit risk; use the appropriate Z-code or surgical excision code (e.g., S061) for those procedures.
Billing Tips
- Ensure you do not bill L708 alongside other surgical procedure codes for the same anatomical site, as this will trigger a duplicate service or 'included in' rejection.
