When to Use

  • Use L709 for the excision of a single benign skin lesion, such as a skin tag or small seborrheic keratosis, when performed via simple excision.
  • Select L709 when the procedure is performed as a minor surgical intervention that does not meet the complexity requirements for a formal excision requiring layered closure under G005 or G006.

Common Pitfalls

  • Billing L709 in addition to a consultation or assessment code (A007/A001) on the same day is often rejected unless a separate, unrelated clinical issue is addressed and documented.
  • Attempting to bill L709 for lesion destruction (e.g., cryotherapy) is incorrect; use Z402 for destruction of benign lesions instead.

Billing Tips

  • If removing multiple lesions, ensure you apply the appropriate multiple procedure rules (50% for the second lesion) to avoid automatic claim rejection.
