When to Use
- Use L714 when performing a simple excision of a benign skin lesion, such as a skin tag or small seborrheic keratosis, that does not require complex closure.
- Select L714 for the removal of a single superficial lesion where the procedure is strictly limited to excision and does not meet the criteria for a more complex excision code like Z001.
Common Pitfalls
- Billing L714 in addition to a minor assessment code (A007) for the same visit is often rejected; ensure the procedure is the primary reason for the visit or use the appropriate diagnostic code to justify the service.
- Avoid using L714 for lesions requiring sutures or layered closure, as these should be billed under the Z-series excision codes which provide higher compensation for the additional work involved.
Billing Tips
- If removing multiple lesions, ensure you apply the appropriate multiple procedure rules (50% for the second, 25% for the third) to avoid automatic rejections for duplicate billing.
