When to Use
- Use L716 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, that does not require formal surgical closure.
- Select L716 when performing a simple shave or scissor excision of a superficial lesion where the procedure is strictly limited to the epidermis or superficial dermis.
Common Pitfalls
- Billing L716 in conjunction with a minor surgical procedure code like Z001 or Z002 is often rejected as the minor assessment is considered included in the excision fee.
- Submitting L716 for lesions requiring deep excision or complex closure is an audit risk; these should be billed under the appropriate Z-code series based on the size and anatomical site.
Billing Tips
- If you perform multiple excisions of the same type, you must bill the first lesion as L716 and subsequent lesions using the appropriate 'add-on' or multiple procedure reduction rules as specified in the Schedule of Benefits.
