When to Use
- Use L721 for the excision of a single sebaceous cyst when the procedure is performed as a minor surgery in the office.
- Select L721 instead of minor assessment codes like A007 when the primary purpose of the visit is the surgical removal of the lesion rather than a consultation or general assessment.
Common Pitfalls
- Billing L721 in conjunction with a minor assessment code (A007) on the same day is generally rejected unless the assessment is for a separate, unrelated clinical issue.
- Submitting L721 for the removal of multiple cysts in the same anatomical region without applying the appropriate multiple procedure rule (50% for the second procedure) will lead to payment discrepancies.

- Ensure the anatomical site is clearly documented in the chart, as this is required to justify the procedure if an audit occurs or if multiple lesions are removed during one session.
