Total laryngectomy is the complete surgical removal of the larynx. Per the Schedule of Benefits, the fee for this service includes any associated laryngoscopy. Additional procedures such as E882 (thyroid lobectomy), E883 (thyroid lobectomy with isthmus and pyramidal tract excision), and E884 (total thyroidectomy) may be claimed in addition when performed.
When to Use
- Use M081 for the definitive surgical excision of the larynx when performed as a primary procedure.
- Use M081 in conjunction with E882, E883, or E884 when a concurrent thyroidectomy is required during the same operative session.
Common Pitfalls
- Do not bill a separate laryngoscopy code, as the fee for M081 explicitly includes any associated laryngoscopy.
- Failure to document the BMI in the permanent medical record will result in a clawback if the E676 morbidly obese patient premium is claimed.
- Incorrectly applying the 85% multiple procedure rule to the add-on codes E882, E883, or E884, which are flat-fee additions and not subject to the same reduction as secondary surgical procedures.
Billing Tips
- Ensure the Injury Severity Score (ISS) is explicitly documented in the medical record to support the E420 trauma premium, as this is a frequent audit trigger.
