Total sternectomy is a major surgical procedure for the complete removal of the sternum. This service includes the work of the surgeon, as well as separate fees for an assistant and anaesthetist. - The assistant fee (M107B) is calculated based on 9 basic units plus time units. The fee for the surgical procedure includes pre-operative and post-operative care for up to two weeks, as outlined in the Surgical Preamble ().

When to Use

  • Use M107A for the definitive surgical removal of the sternum, typically indicated for deep sternal wound infections or malignancy.
  • Select M107A over M106 or M108 when the procedure involves the complete excision of the sternum rather than partial debridement or sternal closure procedures.

Common Pitfalls

  • Failing to bill the first two post-operative visits and the day of discharge visit separately, as these are excluded from the M107A global fee.
  • Incorrectly applying the 15% reduction rule for multiple procedures when the second procedure is for a complication of the initial sternectomy rather than a separate condition.
  • Neglecting to document the Injury Severity Score (ISS) in the medical record when claiming the E420 trauma premium.
