When to Use
- Use M400 to bill for the completion of a mandatory Ministry of Transportation (MTO) medical report when requested by the patient or the MTO.
- Apply this code when the physician is required to complete a formal medical examination report for driver fitness that is not covered under the insured services of the Schedule of Benefits.
Common Pitfalls
- Billing M400 in conjunction with an A007 or other office visit code for the same encounter is considered double-billing and will result in a rejection.
- Attempting to bill M400 for a patient who is exempt from MTO medical reporting fees or when the service is already included in a comprehensive care plan is a common audit trigger.
