Ulnar nerve transposition at elbow - may include exploration, decompression and/or neurolysis. This procedure is listed under both Musculoskeletal System Surgical Procedures and Neurological Surgical Procedures.
When to Use
- Use N189 when performing a formal anterior transposition of the ulnar nerve (subcutaneous, submuscular, or intramuscular) for cubital tunnel syndrome.
- Use N189 as the primary code when the procedure includes the necessary neurolysis or decompression of the nerve at the elbow as part of the transposition.
Common Pitfalls
- Do not bill N189 in addition to a simple ulnar nerve decompression (e.g., N190); N189 is the comprehensive code for transposition and includes the decompression.
- Avoid billing E925 for a primary transposition; this premium is restricted to repeat procedures or specific delayed acute nerve repairs, not routine primary surgeries.
- Failure to document the use of an operating microscope in the operative report will result in the rejection of the E906 premium.
