Posterior spinal decompression, lumbar, one level - bilateral. This procedure includes hemi and total laminectomy, foraminotomy and facetectomy.
When to Use
- Use N512 when performing a bilateral posterior lumbar decompression at a single level involving laminectomy, foraminotomy, and facetectomy.
- Select N512 instead of N511 when the procedure specifically requires bilateral decompression rather than unilateral.
Common Pitfalls
- Billing N512 in conjunction with N511 for the same level is a duplicate billing error, as N512 already encompasses the bilateral scope.
- Attempting to bill for separate foraminotomies or facetectomies is prohibited because these elements are explicitly included in the N512 fee.
Billing Tips
- Append code E361 for every additional lumbar level decompressed beyond the first level covered by N512.
- Ensure the operative report clearly documents the bilateral nature of the decompression to support the N512 claim during an audit.
