- Use Q011 to bill for the completion of a formal, standardized cognitive assessment tool, such as the MMSE or MoCA, when performed as a distinct service.
- Apply this code when conducting a cognitive impairment screening in patients presenting with memory complaints, provided it is not already included in a comprehensive assessment like K030.
Common Pitfalls
- Billing Q011 on the same day as a K030 or K031 is a common rejection, as these codes are considered mutually exclusive for the same cognitive evaluation.
- Failing to document the specific tool used and the resulting score will lead to clawbacks during an audit, as the Ministry requires evidence of the standardized assessment.
