When to Use
- Use Q604 to bill for the completion of the Ontario Ministry of Transportation (MTO) Medical Report for commercial drivers.
- Use this code when the patient is required to undergo a mandatory medical examination for the retention of a commercial driver's license class (A, B, C, D, E, or F).
Common Pitfalls
- Do not bill Q604 for standard G-class license medicals, as these are considered uninsured services and should be billed directly to the patient.
- Avoid billing Q604 in conjunction with an A007 or K030, as the MTO report is a specific administrative service that does not include a clinical assessment fee.
