When to Use
- Use Q734 to bill for the completion of the Ontario Ministry of Transportation (MTO) Medical Report for commercial driver license holders.
- Apply this code when the patient presents specifically for the mandatory periodic medical examination required to maintain a commercial class license (A, B, C, D, E, or F).
Common Pitfalls
- Do not bill Q734 for standard G-class license medicals, as these are considered uninsured services and should be billed directly to the patient.
- Avoid billing Q734 in conjunction with an A007 or K030 assessment on the same day, as the MTO report fee is intended to cover the administrative and clinical work associated with the form.
- Rejections occur if the patient's license class is not verified as commercial, as the Ministry does not cover the cost of private or non-commercial medical reports.
