This service provides for the treatment of two malignant lesions via curettage, electrodesiccation, or cryosurgery on body areas other than the face or neck. The service includes the biopsy of each lesion. As noted on page , payment is conditional upon a pathologist's report being retained in the patient's record. This service is not applicable for pre-malignant lesions. If the resulting wound requires repair by advancement, rotation, transposition, Z-plasty, flap, or graft, the claim should be for the appropriate repair procedure instead of this excision code.
When to Use
- Use R032 when performing curettage, electrodesiccation, or cryosurgery on two distinct malignant lesions located on the trunk or extremities.
- Use this code specifically for confirmed malignant lesions, including in-situ conditions like Bowen's Disease or Lentigo Maligna, when no complex tissue repair is required.
Common Pitfalls
- Billing R032 for pre-malignant lesions like actinic keratosis will trigger an audit risk, as this code is strictly reserved for malignant diagnoses.
- Attempting to bill R032 alongside a separate repair code (e.g., Z427) for the same lesion is a common rejection; if a flap or graft is necessary, you must bill the repair code instead of R032.
- Failing to retain the pathology report in the patient record is a primary cause for payment recovery, as the fee is conditional upon this documentation.

In-situ lesions such as Lentigo Maligna (melanoma-in-situ) and Bowen's Disease (squamous cell carcinoma-in-situ) are considered malignant lesions.
