R058Revision of appendage or tissue re-vascularization involving microanastomosis

OHIP Radiology Code — INTEGUMENTARY SYSTEM SURGICAL PROCEDURES · Schedule of Benefits

Revision of a prior surgical procedure on the skin and subcutaneous tissue. As per the note on page of the Schedule, refer to for guidance on Independent Consideration (IC). Claims for IC services must be submitted with a supporting letter, operative report, and a comparison of the scope and difficulty to other listed procedures. The surgeon, assistant, and anaesthetist fees are all paid on an IC basis.

When to Use

  • Use for complex revisions of microvascular anastomoses where the procedure requires significant dissection and re-establishment of vascular patency that exceeds the scope of standard repair codes.
  • Use when performing a secondary surgical intervention on a previously operated appendage where the complexity of the tissue re-vascularization necessitates an Independent Consideration (IC) assessment.

Common Pitfalls

  • Submitting the claim without a detailed operative report or a comparative analysis against non-IC codes, which leads to automatic rejection or significant payment delays.
  • Failing to explicitly justify the requested fee amount in the supporting letter, which is mandatory for all IC services under GP12.

Billing Tips

  • When drafting your supporting letter, explicitly compare the time, technical difficulty, and risk of R058 to a standard listed procedure like R061 or R062 to provide the medical consultant with a clear benchmark for valuation.
