Revision of R057, appendage or tissue re-vascularization involving microanastomosis with or without micro neuroanastomosis. Surgeon, assistant and anaesthesia fees are all independent consideration (I.C.); see General Preamble .
For Independent Consideration claims, a supporting letter explaining the amount of the fee claimed, an appropriate operative or consultation report, and a comparison of the scope and difficulty of the procedure to non-IC procedures are required.
