R122Preparation of microvascular recipient site for free island skin and muscle flap immediately following ablative surgery

OHIP Radiology Code — INTEGUMENTARY SYSTEM SURGICAL PROCEDURES · Schedule of Benefits

Preparation of microvascular recipient site for free island skin and muscle flap immediately following ablative surgery and when recipient vessels are in the site of the ablation. This service is a component of free island flap procedures. Note: When excision of the lesion and preparation of the recipient site are carried out by different surgeons, the preparation fees should be reduced by 15%.

When to Use

  • Use R122 when preparing recipient vessels specifically within the immediate surgical field of an ablative procedure for a free flap reconstruction.
  • Use this code when the recipient site preparation is a distinct surgical step performed by the primary surgeon or a co-surgeon immediately following the excision of the lesion.

Common Pitfalls

  • Do not bill R122 if the recipient site preparation is considered an inherent part of the primary flap procedure code, as this may lead to claims of unbundling.
  • Failure to apply the 15% reduction when the ablative surgeon and the reconstructive surgeon are different providers will trigger an automatic audit or rejection.

Billing Tips

  • Ensure the operative report explicitly details the microvascular preparation steps to justify R122 as a distinct service from the excision and flap transfer.
