Use R129 when performing microvascular recipient site preparation for free tissue transfer (muscle, tendon, or nerve) in a site distinct from the primary ablative surgery.
Use R129 to bill for the meticulous dissection and preparation of recipient vessels when the reconstruction is performed as a secondary procedure or in a non-ablative context.

Billing R129 instead of R123 when the recipient vessels are located within the immediate site of an ablative surgery, as R123 is the mandatory code for that scenario.
Failing to apply the 15% reduction to the preparation fee when the excision and the recipient site preparation are performed by different surgeons.
Attempting to bill for a second surgical assistant without prior authorization from a medical consultant, as this procedure is not on the pre-approved list for dual assistance.

Ensure the operative report explicitly details the microvascular dissection and preparation of the recipient vessels to support the use of R129 over less complex vascular preparation codes.
