R607 – Slipped epiphysis - closed reduction/traction
OHIP Radiology Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
This service is for the closed reduction of a slipped capital femoral epiphysis (SCFE) using traction. This is a surgical procedure and is eligible for surgeon (suffix A), assistant (suffix B), and anaesthetist (suffix C) fees. The assistant fee is calculated based on 6 base units plus time units. The anaesthetist fee is calculated based on 8 base units plus time units. See the Surgical Assistants' Services (-) and Anaesthesiologists' Services (-) sections of the General Preamble for detailed calculation rules.
When to Use
- Use R607 specifically for the closed reduction of a slipped capital femoral epiphysis (SCFE) performed under traction.
- Select R607 when the procedure is limited to closed manipulation and traction, as opposed to open reduction procedures like R642 which involve formal arthrotomy.
Common Pitfalls
- Billing R607 in conjunction with open surgical fixation codes for the same hip will trigger a rejection for duplicate service or unbundling.
- Failure to document the specific use of traction during the reduction can lead to audit recovery if the procedure is later classified as a simple manipulation.
Billing Tips
- Ensure the anaesthetist and surgical assistant bill their respective base units (8 and 6, respectively) plus time units to maximize the claim value beyond the $387.00 surgeon fee.
