Total cranial vault reshaping is a major craniofacial surgical procedure. The service, as noted with an asterisk (*) on page of the Schedule, includes the harvesting and grafting of bone or cartilage grafts.
When to Use
- Use R677 for comprehensive cranial vault reconstruction, such as total calvarial remodeling for complex craniosynostosis.
- Use this code when the surgical plan necessitates the harvesting and integration of autologous bone or cartilage grafts, as these are inclusive to the procedure.
Common Pitfalls
- Do not bill separately for bone or cartilage harvesting, as the Schedule of Benefits explicitly includes these components within the R677 fee.
- Avoid unbundling R677 with other cranial reconstruction codes; the 'total' nature of the procedure is intended to cover the entire vault reshaping effort.
