OHIP Radiology Code — INTEGUMENTARY SYSTEM SURGICAL PROCEDURES · Schedule of Benefits

Debridement, excision, fasciotomy, and/or grafting for a major burn. This service is paid per unit, where one unit is equal to ¼ hour or major part thereof. Time is calculated from when the physician is first in attendance with the patient in the operating room until they are no longer in attendance. Only payable when rendered in an Operating Room.

When to Use

  • Use for extensive surgical debridement or excision of major burn wounds requiring operating room facilities.
  • Use for split-thickness or full-thickness skin grafting procedures performed in the operating room for major burn management.
  • Use for fasciotomy procedures specifically indicated for compartment syndrome secondary to major burn injury.

Common Pitfalls

  • Failure to document exact start and stop times in the operative report will result in automatic claim rejection.
  • Billing R693 in conjunction with R083 through R093 or R495 on the same day is prohibited and will trigger a rejection.
  • Attempting to bill R693 alongside R691 or R692 for the same patient encounter is a violation of the mutually exclusive billing rules.

Billing Tips

  • Calculate units by rounding to the nearest quarter hour; ensure the total time reflects only the period the physician is actively in attendance in the OR.
  • Always append the appropriate age-based premium or after-hours premium (E409/E410) to the R693 base fee to maximize reimbursement for non-elective procedures.
