R765
R765 – Cardiac massage - open
OHIP Radiology Code — CARDIOVASCULAR SURGICAL PROCEDURES · Schedule of Benefits
Open cardiac massage. This is a surgical procedure and has fee amounts for the surgeon, assistant, and anaesthetist. Note: For closed massage, see the 'Critical Care - Diagnostic and Therapeutic Procedures' section of the Schedule of Benefits.
When to Use
- Use R765 strictly for open cardiac massage where the chest cavity is surgically opened to perform manual compression of the heart.
- Use this code for intraoperative cardiac arrest requiring direct manual cardiac massage when the patient is already undergoing a thoracotomy or sternotomy.
Common Pitfalls
- Billing R765 for closed-chest cardiac massage, which is specifically excluded; closed massage must be billed under the Critical Care section of the Schedule of Benefits.
- Failing to distinguish between open and closed procedures, leading to claim rejections for incorrect service type classification.
Billing Tips
- Ensure the operative report clearly details the thoracotomy or sternotomy approach to support the 'open' designation of the procedure.
- Always verify if the procedure qualifies for non-elective surgical premiums (E409A/E410A) or trauma premiums (E420) if the cardiac arrest occurred in an emergency context.
