R835 – S.V.C. bypass graft
OHIP Radiology Code — CARDIOVASCULAR SURGICAL PROCEDURES · Schedule of Benefits
A surgical procedure involving the creation of a bypass graft for the Superior Vena Cava. This service includes all common and specific elements of a surgical procedure as outlined in the Schedule of Benefits, including but not limited to: - A pre-anaesthetic evaluation and post-anaesthetic follow-up when anaesthesia is provided. - The surgical procedure itself and any related supportive measures. - Normal post-operative care. Supervision of a medical trainee may be eligible for payment under the conditions outlined in to of the Schedule.
When to Use
- Use R835 for the surgical creation of a bypass graft specifically to restore or improve flow in the Superior Vena Cava, distinct from general vascular reconstruction codes like R825 or R826.
- Select this code when the primary surgical intent is the bypass of an SVC obstruction, rather than a simple repair or patch angioplasty of the vessel.
Common Pitfalls
- Billing R835 in conjunction with other major vascular procedures on the same vessel without proper documentation of separate, distinct surgical sites may lead to audit scrutiny for unbundling.
- Failing to include the necessary trauma documentation when attempting to apply the E420 premium for emergency SVC bypasses following significant injury.
Billing Tips
- Ensure the surgical report clearly describes the bypass graft configuration, as this is the primary evidence required to justify R835 over less invasive vascular repair codes.
