Thrombectomy is the surgical removal of a thrombus (blood clot) from an artery or graft. This service is payable only when it is performed as the sole procedure during the operative session.
When to Use
- Use R867 when performing a standalone thrombectomy on an artery or graft where no other surgical procedure, such as an embolectomy or bypass, is performed during the same operative session.
- Use this code for isolated clot removal procedures where the surgical intent is limited strictly to the thrombectomy.
Common Pitfalls
- Billing R867 alongside other surgical procedures performed in the same session will result in automatic rejection, as the code is strictly defined as a sole procedure.
- Confusing R867 with R813 (Embolectomy) is a common audit risk; ensure the clinical documentation clearly distinguishes between the removal of a thrombus versus an embolus.
