Submandibular duct relocation is a surgical procedure performed on the salivary glands and ducts. This service is eligible for surgeon (suffix A), assistant (suffix B), and anaesthetist (suffix C) fees. The fee for the assistant is calculated based on 6 base units plus time units. When to Use

  • Use S057 when performing a formal transposition of the submandibular duct, typically for the management of ductal stenosis or stricture.
  • Use S057 when relocating the duct to the oral cavity floor as a primary surgical intervention for salivary flow obstruction.

Common Pitfalls

  • Do not bill S057 in conjunction with simple ductal dilation or sialendoscopy procedures, as these are distinct services.
  • Do not assume a second assistant is automatically payable; S057 is not on the pre-approved list, and you must submit a letter of justification to the Ministry for approval.
  • Failure to document the Injury Severity Score (ISS) in the medical record will lead to the rejection of the E420 trauma premium.

Billing Tips

  • Ensure the anaesthetist's claim includes the base units (7) plus time units, and verify that any additional units for ASA status or BMI are calculated according to the rules in GP97.
  • When billing for patients under 16 years of age, verify the correct age-based premium (U1, U5, or U16) is applied to both the surgeon and assistant fees to maximize the claim value.
