S177Intestinal obstruction (mechanical) - with resection

OHIP Urology Code — DIGESTIVE SYSTEM SURGICAL PROCEDURES · Schedule of Benefits

A one-stage surgical procedure for the correction of a mechanical intestinal obstruction, which includes the resection of a portion of the intestine. This service is listed under the Digestive System Surgical Procedures section. Note: If performed as a staged procedure, different billing rules apply. Refer to the Surgical Preamble for details on staged procedures.

When to Use

  • Use S177 for a single-stage mechanical intestinal obstruction resection where the bowel is resected and primary anastomosis or closure is achieved in one operative session.
  • Use S177 when the obstruction is caused by intrinsic mechanical factors (e.g., stricture, intussusception) requiring excision, distinguishing it from S175 or S176 which typically involve simple lysis of adhesions or non-resectional procedures.

Common Pitfalls

  • Billing S177 alongside lysis of adhesions (S175) is generally considered bundled; ensure the resection is the primary procedure and not incidental to a more complex procedure that should be billed under a different code.
  • Failing to document the specific mechanical nature of the obstruction can lead to audit rejections, as S177 is strictly for mechanical obstruction and not for functional or paralytic ileus.
  • Attempting to bill S177 for a staged procedure (e.g., initial diversion followed by later resection) violates the Surgical Preamble; staged procedures require adherence to the specific billing rules outlined in SP2.

Billing Tips

  • Always verify if the procedure qualifies for E409A or E410A based on the exact start time of the surgery, as these non-elective premiums significantly increase the base fee for S177.
  • If the patient meets the criteria for the Trauma Premium (E420), ensure the Injury Severity Score is explicitly documented in the chart, as this is a frequent point of audit focus for surgical claims.
