Closure of colostomy or enterostomy, performed with or without resection and/or anastomosis.
When to Use
- Use S185 for the elective takedown of a loop or end colostomy where the primary goal is the restoration of bowel continuity.
- Use S185 when the procedure involves a formal resection and primary anastomosis of the bowel segments at the site of the previous stoma.
- Use S185 for enterostomy closures, including ileostomy takedowns, provided the procedure is limited to the site of the stoma.
Common Pitfalls
- Do not bill S185 in conjunction with S312, as these are restricted codes and will result in a claim rejection.
- Avoid billing S185 if the procedure involves an extensive bowel resection or complex abdominal exploration beyond the stoma site, which may require a different, higher-valued surgical code.
- Failing to document the specific type of stoma (e.g., loop vs. end) can lead to audit scrutiny regarding the complexity of the closure performed.
