Percutaneous trans-hepatic catheter drainage of obstructed bile ducts including daily supervision and including percutaneous cholangiogram and catheterization to duodenum if achieved.
When to Use
- Use S233 for the primary placement of a percutaneous trans-hepatic biliary drainage (PTBD) catheter in patients with obstructive jaundice.
- Use this code when the procedure includes the initial percutaneous cholangiogram and the attempt to traverse the obstruction into the duodenum.
Common Pitfalls
- Do not bill S233 in conjunction with S234, as S233 is intended for the initial drainage procedure and S234 covers subsequent exchanges or revisions.
- Avoid billing daily hospital care codes (A003/A005) for the same day as S233, as the code description explicitly includes daily supervision for the procedure duration.
- Ensure the procedure is not unbundled; the cholangiogram and catheterization are inclusive and cannot be billed as separate diagnostic imaging or procedural codes.
Billing Tips
- If the patient is under 16 years of age, ensure the appropriate age premium is applied to the S233 fee to capture the 10-30% increase as defined in the Schedule of Benefits.
