S241 – Sphincterotomy(ies) under local anaesthesia
OHIP Urology Code — DIGESTIVE SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
A surgical procedure listed under the Digestive System section, involving one or more sphincterotomies performed under local anaesthesia. Payment for surgical assistant (suffix B) and anaesthetist (suffix C) services are calculated based on basic units plus time-based units as defined in the General Preamble. This service is performed under local anaesthesia.
When to Use
- Use S241 for a standard sphincterotomy performed in an office or clinic setting strictly under local anaesthesia.
- Use this code for the surgical division of an anal sphincter muscle when the procedure does not require general or regional anaesthesia.
Common Pitfalls
- Do not bill S241 if the procedure is performed under general or regional anaesthesia; S243 is the appropriate code for procedures involving more complex anaesthetic management.
- Avoid billing S241 in conjunction with a consultation or assessment code on the same day unless the assessment is for a separate, unrelated clinical problem.
- Failure to document the use of local anaesthesia specifically can lead to audit recovery if the claim is flagged for review against higher-valued surgical codes.
Billing Tips
- Ensure that if you are acting as the primary surgeon, you do not also bill for an anaesthetic fee, as the code explicitly defines the service as being performed under local anaesthesia.
- If the procedure is performed after hours, ensure you append the appropriate E409 or E410 premium to the S241 base fee to capture the 50% or 75% increase respectively.
