Operative repair of an umbilical hernia in a child. The fee for this service is for the surgeon (suffix A) and includes all associated pre-operative and post-operative care as described in the General Preamble. This service may be eligible for various premiums such as age-based, after-hours (E409, E410), and trauma (E420). When applicable, fees for an assistant (suffix B) and anaesthetist (suffix C) are calculated separately based on basic and time units, and have their own sets of applicable premiums.

When to Use

  • Use S333 for the primary operative repair of a pediatric umbilical hernia in patients under 18 years of age.
  • Use S333 in combination with E725 if the procedure is specifically documented as a repair of a recurrent umbilical hernia.

Common Pitfalls

  • Billing E764 alongside S333 is a common rejection, as E764 is intended for umbilical hernia repairs performed during other major abdominal surgeries and is mutually exclusive.
  • Attempting to bill S333 for patients 18 years or older will result in rejection; use the appropriate adult hernia repair codes instead.
  • Failure to document the specific Injury Severity Score (ISS) in the medical record will lead to the rejection of the E420 trauma premium.

Billing Tips

  • Ensure you apply the correct age-based premium (AGE_PREMIUM_2-4 or AGE_PREMIUM_5-15) as a percentage increase to the base fee to maximize reimbursement for pediatric patients.
