Nephrolithotomy or pyelolithotomy - open or laparoscopic
When to Use
- Use S405 for the surgical removal of renal calculi via open nephrolithotomy or laparoscopic pyelolithotomy when endourological approaches like S401 or S403 are not feasible.
- Select S405 when performing a formal open or laparoscopic incision into the renal pelvis or parenchyma specifically to extract a stone, distinguishing it from simple ureterolithotomy (S408).
Common Pitfalls
- Billing S405 in conjunction with J069 is prohibited as the Ministry considers these services mutually exclusive for the same operative session.
- Submitting S405 for percutaneous nephrolithotomy (PCNL) is a common error; ensure the procedure meets the open or laparoscopic definition rather than percutaneous endoscopic techniques.
Billing Tips
- If the procedure is performed as a non-elective emergency between 00:00 and 07:00, ensure E410 is appended to capture the 75% procedural premium.
