When to Use

  • Use S569 for a dorsal slit procedure performed on a child or adult to relieve phimosis or paraphimosis.
  • Select S569 when the patient is older than the infant age threshold, as S568 is strictly reserved for infants.

Common Pitfalls

  • Billing S569 for a newborn or infant will result in rejection; ensure the patient's age aligns with S567 or S568 respectively.
  • Attempting to bill an assistant fee directly with S569 will be rejected, as there is no assigned assistant unit in the Schedule of Benefits.

  • If an assistant is medically necessary for this procedure, you must submit M400B along with a letter of justification to a medical consultant for manual review.
