This surgical procedure involves the emergency detorsion (untwisting) of a torsed testis or appendix testis to restore blood flow. Following the reduction of the torsion, an orchidopexy is performed, which involves surgically securing the affected testicle (and often the contralateral one as a preventative measure) to the scrotal wall. The fee for this service is comprehensive and covers the procedure for one or both sides if required during the same operation.
When to Use
- Use S600 for the emergency surgical detorsion and fixation of a testis presenting with acute torsion.
- Use S600 when performing a prophylactic orchidopexy on the contralateral testis during the same operative session as the primary detorsion.
Common Pitfalls
- Do not bill S600 in addition to other scrotal exploration codes, as S600 is a comprehensive fee that includes the exploration and fixation.
- Avoid billing S600 for simple scrotal exploration if no torsion is found; in cases of negative exploration, use the appropriate exploratory code instead.
Billing Tips
- Ensure you apply the age-based fee modifier for patients under 16 as per GP64, as this is a common pediatric emergency procedure.
- Always append the appropriate after-hours premium (E409) if the procedure is performed outside of standard daytime hours, as this is typically an urgent, non-elective surgery.
