Transurethral resection of prostate and may include cystoscopy, meatotomy, dilatation of stricture, internal urethrotomy or vasectomy.
When to Use
- Use S655 for the primary transurethral resection of the prostate (TURP) to treat benign prostatic hyperplasia (BPH).
- Use S655 when the procedure includes ancillary steps such as cystoscopy, meatotomy, urethral dilatation, or internal urethrotomy, as these are considered bundled and not separately billable.
Common Pitfalls
- Do not bill S655 for repeat procedures performed within one year of a previous prostatectomy by the same surgeon; use S654 for residual or regrowth tissue instead.
- Do not attempt to unbundle and separately claim cystoscopy or urethral stricture management when performed during the same operative session as S655, as these are explicitly included in the fee.
- Failure to document the specific clinical indication for a repeat procedure can lead to audit scrutiny if S655 is used when S654 is the appropriate code for post-prostatectomy recurrence.
Billing Tips
- Ensure that any applicable age-based premiums or after-hours premiums (E409/E410) are appended to the claim if the patient meets the specific age or timing criteria defined in the Schedule of Benefits.
- If a bilateral orchidectomy is performed within 10 days of the TURP for prostate carcinoma, apply the multiple procedure rules outlined in the Surgical Preamble (SP2) to ensure correct payment.
