When to Use
- Use S778 when performing a formal presacral neurectomy for the treatment of chronic pelvic pain, specifically dysmenorrhea refractory to medical management.
- Select this code when the procedure is performed in conjunction with an ovarian neurectomy, as the fee is inclusive of both components.
Common Pitfalls
- Do not bill S778 in addition to other pelvic floor or uterine repair codes unless the procedures are distinct and performed through separate incisions or distinct anatomical sites.
- Avoid billing S778 if the procedure is performed as part of a more comprehensive pelvic surgery where a higher-valued procedure code covers the presacral dissection, as this may trigger a duplicate service rejection.
Billing Tips
- Ensure the operative report explicitly details the identification and resection of the superior hypogastric plexus to justify the use of this specific surgical code over more generic pelvic exploration codes.
