- Use T374 for the surgical excision of a malignant lesion of the skin on the face, ears, or neck where the diameter of the lesion is between 1.1 cm and 2.0 cm.
- Select T374 when the procedure involves a full-thickness excision requiring primary closure, as opposed to simple destruction codes like S062 or S063.

- Billing T374 alongside a consultation or assessment code on the same day is often rejected unless a distinct, unrelated clinical issue is documented and appended with the appropriate diagnostic code.
- Submitting T374 for lesions smaller than 1.1 cm is a common audit trigger; ensure the pathology report confirms the measured diameter of the excised specimen.
