- Select T488 over smaller excision codes like T486 or T487 when the size of the lesion necessitates a more extensive surgical procedure and complex closure.

- Billing T488 for benign lesions is a frequent audit trigger; ensure the pathology report confirms malignancy to support the fee.
- Attempting to bill an additional excision code for the same site is prohibited; T488 is a comprehensive fee that includes the primary excision and standard closure.
