- Select T646 instead of the general excision codes (e.g., T640-T644) to account for the higher complexity and cosmetic sensitivity of facial anatomical sites.
Common Pitfalls
- Billing T646 for benign lesions will result in a rejection or audit recovery; ensure the pathology report confirms malignancy.
- Submitting T646 in conjunction with a minor procedure code for the same lesion is considered unbundling and will be rejected by the Ministry.
