W050 – Special community medicine consultation

A special community medicine consultation rendered to a patient in a long-term care institution. This service is subject to the same conditions as A050. As a consultation, it requires a written request from a referring physician, nurse practitioner, or dental surgeon in connection with an insured dental procedure in hospital. A copy of the written request must be kept in the medical record. The request must identify the consultant, the referring practitioner, and the patient, and set out relevant information specifying the service(s) required. The consultant must prepare a written report for the referring practitioner. See to for full consultation requirements.
When to Use
- Use W050 when providing a formal consultation for a patient residing in a long-term care facility, specifically when the request originates from a physician, nurse practitioner, or dental surgeon.
- Select W050 instead of A050 when the service location is explicitly a long-term care institution, as it is the designated code for that setting.
Common Pitfalls
- Billing W050 without a documented, written referral request in the chart will lead to a clawback, as it is subject to the same strict consultation requirements as A050.
- Failing to include the referring practitioner's billing number in the consultation report or medical record is a frequent cause for audit rejection.
- Using W050 for a follow-up visit is an error; follow-up care in long-term care should be billed using the appropriate subsequent visit codes, not a consultation code.

A copy of the written request for the consultation, signed by the referring physician, nurse practitioner or dental surgeon must be kept in the consulting physician's medical record. In a long-term care institution where common medical records are maintained, the written request may be contained on the common medical record.
The request identifies the consultant by name, the referring physician, nurse practitioner or dental surgeon by name and billing number, and identifies the patient by name and health number.
The written request sets out the information relevant to the referral and specifies the service(s) required.
The consultant must prepare a written report (including findings, opinions, and recommendations) to the referring physician, nurse practitioner or dental surgeon.
Subject to the same conditions as A050.
