W771 – Certification of death

Certification of death for a patient in a chronic care hospital, convalescent hospital, nursing home, or home for the aged. As per page , this service is included as a component of the W010 (Monthly Management of a Nursing Home or Home for the Aged Patient) fee. It is not separately payable in a month where W010 is claimed, unless the service is performed in conjunction with a special visit.
When to Use
- Use W771 when certifying a death in a long-term care facility if you are not the physician responsible for the W010 monthly management fee.
- Use W771 in conjunction with a Special Visit Premium (e.g., K990, K992) if you are the W010 provider but the death certification occurs during an urgent, non-elective visit outside of your routine management duties.
Common Pitfalls
- Claiming W771 as a standalone service while also billing W010 for the same patient in the same month will result in an automatic rejection.
- Attempting to bill W771 for death certification in a private residence or acute care hospital, as this code is strictly restricted to chronic care, convalescent, or long-term care settings.
Billing Tips
- Always append the appropriate Special Visit Premium (e.g., K990) to W771 when the death certification requires an urgent, non-elective trip to the facility to ensure the service is not considered part of the W010 monthly management fee.
