This service includes both a technical component (performing the X-ray) and a professional component (interpretation of the images). The fee listed in the Schedule is a total fee, which is comprised of the professional and technical components.
When to Use
- Use X218 when a clinical assessment requires a comprehensive wrist series consisting of four or more distinct radiographic projections to evaluate complex fractures or suspected occult pathology.
- Select X218 over X219 (Wrist - three views) when the clinical protocol or injury complexity necessitates additional views, such as scaphoid or carpal tunnel projections, to reach a diagnostic threshold.
Common Pitfalls
- Billing X218 when only three views were performed; if the imaging protocol does not meet the four-view minimum, the claim is subject to recovery upon audit.
- Failing to split the claim into technical (X218B) and professional (X218C) components, as the Schedule requires separate submissions for these distinct elements.
Billing Tips
- Ensure the radiology report explicitly lists the four or more views captured to substantiate the claim if an audit occurs.
- When billing the professional component (X218C), ensure the interpretation is documented and signed to satisfy the professional component requirements.

The claim for the technical component is submitted using the fee schedule code with the suffix B and the claim for the professional component is submitted using the fee schedule code with a suffix C.
