Level 3 - Complex Treatment Planning is the service of planning a complex course of radiotherapy for a patient, paid per patient per course of treatment. Eligibility requires that the treatment plan must include one or more of the specific complex treatments or planning techniques listed on page of the Schedule.
When to Use
- Use X312 when the treatment plan involves complex techniques such as intensity-modulated radiation therapy (IMRT) or stereotactic body radiotherapy (SBRT) as defined in the Schedule.
- Apply this code when the clinical complexity necessitates a comprehensive multi-field planning process that exceeds the requirements for standard X311 planning.
Common Pitfalls
- Billing X312 for standard palliative radiotherapy plans that do not meet the specific technical complexity criteria outlined in Section C3.
- Submitting X312 more than once per patient per course of treatment, as this code is strictly limited to a single occurrence for the entire planning phase of that course.
